How to use this form.

With this form you can submit the results from assessment A1 and A2.

A couple notes:

• You don't need to fill in both scores at the same time.
  You can fill in the result for A1 and come back later and fill in the result for A2.
• You can only fill in a result for A2 if there is a record in our database for A1 or if you submitting both results at the same time.
• You can not change your scores once they are submitted.

If you need any help filling out this form you can contact us schools@fitforsport.co.uk

The system will try and suggest school names from previous entries but you can always type it in yourself.
